PACE Baton Rouge (Programs of All-Inclusive Care for the Elderly) is a non-profit health and supportive services program designed to assist seniors who want an alternative to nursing home placement. The mission of PACE Baton Rouge is to enable the aging population to live with dignity in their communities. With PACE Baton Rouge’s assistance, participants can remain in the secure surroundings of their own homes and communities while providing family members and caregivers with much needed professional guidance, physical relief and emotional support.

Dietitian will be responsible for providing nutrition care and education to PACE participants and caregivers and clinical staff as necessary. Evaluate the participant’s nutritional status, develop and implement nutritional care plans, assess nutrient requirements, recommend nutritional prescriptions, evaluate participant tolerance and effectiveness of nutritional care, and communicate nutritional data and information to the physicians and other health professionals.

Education: Bachelor’s Degree from an ADA accredited institution with a major in nutrition. Must be ADA registered. CDE status preferred but not required, should be CDE eligible within 24 months. Experience: Must have one year experience working with the frail and/or elderly. Special Training: Able to teach, think analytically and relate to geriatric participants. Knowledge of clinical dietetics mandatory

Take Shape for Life (TSFL) is an Optimal Health Program designed and led by health professionals.  We use pharmaceutical grade Medifast meal replacements as a tool to help people lose weight quickly and safely while teaching them habits of health so they can transition off the meals, maintain their weight and get rid of obesity related disease and medications.  The Medifast pharmaceutical grade meal replacements are used at Johns Hopkins University in their weight management clinic.  These meals are nutrient rich and balanced in carbohydrate, protein and fat.  Start up costs for a health professional to become a TSFL health Coach is minimal.   Full training and support is provided to help you implement and run the program.  The coaching can be done from your office or home by phone or email.  This position may be done part-time or full-time.
